Kickback at the Y Guest Waiver

Kickback at the Y Participants:

• This event is open to youth 14 years old to 18 years old.
• Please fill out the guest waiver prior to arriving for the event.
• All participants under the age of 18 will need a parent/guardian's signature on the form as well as a photo of the parent/guardian's ID submitted with the form in order to participate.
• If the online form does not work, please print and fill out the guest waiver attached here. Once filled out, please bring the completed form and a form of ID (parent/guardian's ID if participant is under the age of 18) to the front desk and let the membership specialist know that it is for the KBBH Opioid Use Prevention Event prior to the event.
